Yes, mistakes have happened from our end. However, there are time where packages are tampered with while they are enroute. This is very upsetting to us because it is a big inconvenience to our customers and we have to pay for the replacement item and the shipping cost to resend the 'missing' item. Katprr is right, no CC info is put on the packing slips. Four the thousands and thousands of packages we have shipped, this post if the first I have heard of boxes 'popping' open. The priority mail boxes are self-adhesive and do not require taping. This adhesive obviously failed in these occasions.
