I am not saying this is always true, but sometime a huge shift like this: super girly as a child and then boyish/baggy clothes as a teen can be a sign that something happened. I went through this after I was rapped as a young teen. I mistakenly thought that because I dressed girly, I got was I was "asking for" so if I dressed boyish and in baggy clothes I was no longer asking for it, and it would not happen again. She’s had some other issues OCD and maybe something else so I believe she sees someone regularly.
